Your COVID-19 vaccination record

We typically receive vaccination data within 2 to 4 weeks of the vaccination date, depending on where you received the vaccine. Your vaccine information from vaccination providers should be available through this tool. At this time, UnitedHealthcare is not receiving all vaccination data. Depending on where you received your vaccine, the vaccination data might not yet be available.

If your vaccine data is missing and you wish to report it, please use our self-reporting tool.

Should your vaccination data be received in the future, we will replace your self-reported data with the data received through your vaccination provider.

If we receive multiple records for your vaccination, we will prioritize the data as follows:

1. Official claim (medical or pharmacy)
2. Records from federal and state agencies
3. Provider electronic medical record

UnitedHealthcare uses available claims data as well as information from federal and state agencies and vaccination providers, to populate your vaccination record. We continue to work closely with federal and state authorities, and vaccination providers regarding obtaining vaccination records of our members.

Our vaccine pass program receives information from multiple resources such as healthcare claims records under your UnitedHealthcare insurance plan, records your healthcare provider has shared with us, and state and federal vaccination registries. This includes any booster information if those sources provide it to us.
